What can the SOAR Team do for an individual who experiences a sudden mental health status change and is behaving in an unusual manner?
|
| The SOAR Team will take a referral and assess the individual. Often
times, a sudden change of mental status is an indicator of problems such as poor
nutrition, dehydration, isolation, physical issues, and possibly grief or loss. The
team will assess the situation and then make plans to connect the individual with the
appropriate services. Follow-up visits would then be provided.
