FAQ - Senior Outreach Assessment Response (SOAR)
Question:
What is the SOAR Program?
Answer:
SOAR stands for Senior Outreach Assessment Response. Basically, it is a program that provides outreach and needs assessment of older adults that are having particular difficulties or are isolated from others.

Question:
What type of assessment does the SOAR Team typically provide?
Answer:
The SOAR Team will go to the home of a Senior Adult to assess the individuals needs and also the Mental Status of the individual.

Question:
Does the SOAR Team work cooperatively with the Mental Health Department?
Answer:
Yes. After an assessment, should it be determined that an individual would benefit from Mental Health Services, a referral will be made on behalf of the person.

Question:
What can the SOAR Team do for an individual who experiences a sudden mental health status change and is behaving in an unusual manner?
Answer:
The SOAR Team will take a referral and assess the individual. Often times, a sudden change of mental status is an indicator of problems such as poor nutrition, dehydration, isolation, physical issues, and possibly grief or loss. The team will assess the situation and then make plans to connect the individual with the appropriate services. Follow-up visits would then be provided.

Question:
Does the SOAR Team provide counseling or case management services?
Answer:
Yes, the team provides brief case management and counseling. However, if the individual has a chronic mental illness, he/she is referred to the Mental Health Department.

Question:
How do I know when it is appropriate to contact SOAR?
Answer:
If there is any uncertainty, make a telephone call referral. The SOAR representative will discuss the situation with you and determine if it is an appropriate referral. If it is not appropriate, information and referral of resources will be provided to you.
